AK9C Acknowledgement of Waiver
General: I, the undersigned, understand that boarding and doggie daycare is a service that provides safe and secure care for my pet but is not without risk. American K9 Country Inc. (Ak9C) ensures quality pet care but understands and states that pets may get stressed and rambunctious at times, and that WE CANNOT BE HELD RESPONSIBLE FOR ANY INJURIES AND OR ILLNESSES THAT MAY OCCUR DURING OR RESULTING FROM THEIR BOARDING AND/OR DAYCARE STAY.
Socialization: I, the undersigned, understand that dogs are encouraged to socialize & play while at the AK9C facility & that doggie daycare is a wonderful service that provides great fun and socialization for my dog but is not without risk. American K9 Country Inc. takes great care in supervising the dogs at play but understands and states that dogs do get rambunctious and rough at times, and that we cannot be held responsible for any injuries that may occur as a result. BY SIGNING THIS FORM, YOU THE OWNER OR AGENT OF SAID DOG, ACKNOWLEDGE AND ACCEPT THE TERMS OF THIS RELEASE AND HOLD HARMLESS AK9C FOR ANY INJURY, ILLNESS, ESCAPE OF DOG OR ANY LIABILITY WHATSOEVER TO YOUR DOG WHILE IN AK9C’S CARE. I UNDERSTAND THAT MY AGREEING TO THE FOREGOING AND, SPECIFICALLY, MY AGREEMENT NOT TO HOLD AK9C RESPONSIBLE FOR THE PAYMENT OF VETERINARY BILLS AND OR RESTITUTION IS THE ESSENCE OF THIS AGREEMENT AND THAT AK9C WOULD NOT AGREE TO CARE FOR MY DOG WITHOUT THIS INDUCEMENT TO DO SO.
Health: I understand that AK9C reserves the right to refuse any service or use of facilities to any pet that acts aggressively, appears unhealthy, is NOT up to date on Rabies, Bordetella, Distemper & Parvo vaccinations (veterinary proof is required) or is otherwise a danger to themselves, other animals or AK9C Staff. Also any pet entering the AK9C facility MUST be free of external & internal parasites, use of Veterinary Recommended Flea & Tick Preventative (Frontline, Advantix, Nexguard etc) IS MANDATORY! If a pet entering the AK9C facility is found carrying disease and/or parasites including fleas, a surcharge of $85 will be billed to the owner for facility cleaning. Collar Policy: All dogs must wear a flat collar. This collar must be identified with their name either by means of a tag or the name embroidered directly on the collar. All daycare dogs must wear a collar with identification- there are no exceptions. We do not allow choke, martingale, chain, electronic, flea/tick or pinch collars for boarding or daycare dogs. If your dog arrives for daycare without identification on their collar, a tag must be purchased from Ak9C for a $10.00 fee at the time of drop off. Sorry there will be no exceptions to the rule.
Hours: I understand that although AK9C is staffed 24 hours a day, I may only pick up & drop off my pet during the designated office hours: Monday-Friday 6:30am-6:30pm Saturday & Sunday 9am-4pm. There will be NO exceptions to this rule.
Check Out: Boarding check out time is 9:00am, any pet checked out after this time shall incur an additional days charge. I understand that if I elect to utilize "Express Check-out" my pet MUST be a daycare candidate & that I must call & check out my pet BEFORE 9am on the last day of boarding. My credit card will be directly billed for his/her boarding & daycare stay at this time. If I do not call before 9am, my dog's kennel will be held for the day therefore incurring both a boarding & daycare charge.
*NOTE: Any daycare dog that is NOT picked up before 6:30pm shall be considered “Boarding” and spend the night, which will incur one nights boarding charge at the cost of owner. We do NOT pro-rate partial boarding days.
Abandonment: I understand that if I fail to contact AK9C regarding a change in my pet’s departure date and I remain unable to be reached either by phone or mail for a period greater than 7 days after the scheduled pickup date, my pet will be considered abandoned and property of AK9C. AK9C may then dispose of or place the dog/cat in whatever manner necessary. [According to NH RSA 437:19]
Payment: I understand that all fees MUST be paid in full at the time of pick up or prior to the departure of animal. I also acknowledge that if I refuse to pay the just fees & charges incurred by my pet, AK9C shall retain custody of my animal until ALL charges are paid. If payment arrangements are not made with AK9C management within a period of 7 days after my pet’s scheduled departure then I shall forfeit my pet’s ownership and it will be considered abandoned and property of AK9C. [According to NH RSA 437:19]
Emergency Care: If in my absence my pet shall become injured, ill or otherwise requiring veterinary attention deemed by AK9C, AK9C is fully authorized in its discretion to consult and or utilize either the services of my veterinarian or the services of any local veterinarian for the treatment of my pet. I understand that all emergency veterinary charges that my pet may incur while in the care at AK9C shall be directly billed to my credit card. I also authorize AK9C to have full access to my dog’s veterinary records and past medical history.
Cancellations: I understand that AK9C requires 48 hour notice prior to my pet’s scheduled date of arrival to cancel a reservation without incurring a fee (*with the exception of HOLDAYS & School Vacation weeks-where there is NO grace period & ANY cancellation will be charged $75/pet). Anyone failing to cancel their pet’s reservation without a 48-hour notice shall incur a $75.00 cancellation charge. I authorize this charge to be directly applied to my credit card.
Credit Card Charges: I authorize AK9C to maintain my credit card number on file and charge it accordingly in respect to emergency veterinary care, as well as in the case of “Cancellations” as referred to above.
